====== Truro - Report Riders and Codes ======

This page exists to catalogue the specific riders, admin codes and other blocks of text Truro would like added to reports, and the circumstances in which they should be used. Truro generally doesn't specify lots of these, but there are a few ones that they are keen on.

 ===== Aortic Valve Calcification =====

If moderate or severe aortic valve calcification is present add the below text. There are no strict rules on how to grade this, but the linked document gives some representative pictures.

''Moderate/Severe aortic valve calcification is noted. This may indicate the presence of aortic valve stenosis. Consider echocardiography if clinically appropriate.''

===== Coronary Calcification =====

If any degree of coronary calcification is present add the below text, which can be done with the "get coronary calcification command". There are no strict rules on how to grade this, but the linked document gives some representative pictures.

''(Mild / moderate / severe) coronary artery calcification is noted. If clinically appropriate we would advise reviewing addressable cardio-vascular risk factors with the patient in primary care. Further testing or assessment for those without symptoms may not be necessary or recommended (see guidance on the RMS website). Please only refer for assessment those with suspected cardiac chest pain following the RACPC guidance / pathway which can be found on eRS and RCHT website.''

===== Stroke Head and Angiogram Scans =====

For acute stroke head and angiograms, in addition to a normal report Truro would like you to use the "auto stroke" CRIS VR command, which pops-up a dialogue box with some questions and auto fills some text dependant on that.

===== Fragility Fractures =====

If a definite fragility fracture is present (e.g. vertebral body, surgical neck of humerus in low trauma ect.) the "get fragility fracture" command should be used, which inputs the below text:

''Appearances suggest a high risk of fragility fracture - in patients between the age of 50 and 75 referral for Bone Densitometry scanning is suggested. NICE guidelines recommend presumptive treatment in patients over 75 following a fragility fracture.''
