Derriford On-Call Shifts

Derriford On-call shifts vary but can be pretty hectic, with lots of calls, scans with a lot of complexity, and consultants that can be a bit distant.

Weekend have now moved to a single 0845 - 2115 shift covered by two registrars, with varying duties according to seniority.

Both registrars are expected to check in at the start of the day for the imaging production board meeting to divvy out work and see the state of play.

Following this the registrars are expected to cover the phones through netcall, and report all category 3 and 5 IP CT as well as MRI Lumbar spines until 2100.

Additionally there are CT head and plain film duties to be covered in order as able, depending on who is on:

  • If post FRCR registrar - IP and ED CT heads (including stroke, post-contrast)
  • If post CXR redspot - ED and IP CXR 0000 - 2359 Saturday only.
  • If post ED MSK redspot - ED and IP MSK both Saturday and Sunday.

Registrars should split the first two allowed duties between them.

There is no obligation to cover IP category 1 CTs.

Hours: 1700 - 2130

Main duty is covering the phone with some reporting as able. From 1700-1900 you are supported by the EWD consultant and need to touch base with them in terms of what reporting to do. From 1900-2130 it moves to the on-call consultant.

In order to avoid scans going to PROC in the evening vetting changes a bit:

Scans vetted after 1900 should be assigned to the PROCREP silo if urgency is 3/5, and left in IP silo if 1.

At 1900 the EWD consultant is supposed to go through the outstanding 3/5 scans requested before 1700 and triage them to one of:

  • Not required out of hours - downgrade urgency to 1.
  • Required out of hours, delayed due to hospital delays - send to medica.
  • Required out of hours, delayed due to patient condition - put in PROCREP silo for PROC reporting
